61.65 -  250 nm Cross-country for the FAA Instrument rating.
(i) Three hours of instrument flight training from an authorized (FAA certified and current) instructor in an airplane that is appropriate to the instrument-airplane rating within 2 calendar months before the date of the practical test; and
(ii) Instrument flight training on cross country flight procedures, including one cross country flight in an airplane with an authorized instructor, that is performed under instrument flight rules, when a flight plan has been filed with an air traffic control facility, and that involves— NOTE: Authorized means a current FAA Instrument Instructor. Safety Pilots don't count!
(A) A flight of 250 nautical miles along airways or by directed routing from an air traffic control facility;
(B) An instrument approach at each airport; and  An means one, 1, un, uno etc.... The word each implies more than one airport.
(C) Three different kinds of approaches with the use of navigation systems. i.e, example ILS, VOR & NDB. NOT THE SAME APPROACH AT THREE DIFFERENT LOCATIONS and (B) DOES NOT MEAN THREE DIFFERENT APPROACHES AT THE SAME AIRPORT.
(B) says An instrument approach at each airport; and
(C) saysThree different kinds of approaches with the use of navigation systems.
* So, If it says an instrument approach at each airport and indicates three different approaches with the use of navigation systems, then in plain English it means 3, count them. 3 different approaches at three different airports!
* Also be aware of the FAA’s definition of a cross-country. For  it to be a cross-country at all, you must have landed somewhere other than the point of departure!
 
Last but not least,IFR does not mean IMC! Not only does the FAA criteria say so but so does the JAR FCL criteria. Not in the US, Europe or anywhere else in the world! Also,you cannot, I REPEAT you cannot be IFR from start up to shut down.
To fulfill the requirement for an FAA Instrument practical flight test you must have 40 hours of actual or simulated IMC time. 15 hours MUST be with a current and authorized FAA Instrument Instructor and you will be required to prove it to the examiner. Apart from the 250 NM IFR cross-country, the actual IFR time is irrelevant.
